🔬 IVF Basics

IVF (In Vitro Fertilization) is a fertility treatment where eggs are retrieved from the ovaries and fertilized with sperm in our laboratory. The resulting embryo is cultured for 3–5 days and then transferred to the uterus. At Akanksha IVF Centre, every step is monitored by Dr. K.D. Nayar using the latest protocols including time-lapse embryo imaging.

In standard IVF, eggs and sperm are placed together and fertilization occurs naturally. In ICSI (Intracytoplasmic Sperm Injection), a single healthy sperm is directly injected into an egg. ICSI is recommended for severe male infertility, low sperm count, poor motility, or when previous IVF cycles have failed to fertilize. We combine ICSI with Microfluidic Sperm Sorting (MFSS) for best results.

IUI (Intrauterine Insemination) is a simpler, less expensive procedure where washed sperm is placed directly into the uterus around ovulation. It does not involve egg retrieval. IVF is more advanced — eggs are retrieved, fertilized externally, and the resulting embryo is transferred. IUI is typically recommended first for milder infertility cases; IVF is used for more complex situations.

As per India’s ART Act 2021, women between 21–50 years may seek IVF treatment. Success rates are highest under 35 and decline gradually with age. Women above 40–42 are often advised to consider donor eggs for better outcomes. Dr. K.D. Nayar evaluates every patient individually and advises the most suitable approach.

IVF involves daily hormone injections for 10–12 days, which can cause mild bloating or discomfort. The egg retrieval is performed under sedation/anaesthesia, so it is not painful. The embryo transfer is a simple painless procedure similar to a Pap smear. Most patients report the experience is more manageable than they expected.

PGT (Preimplantation Genetic Testing) tests embryos for chromosomal abnormalities (PGT-A) or inherited genetic diseases (PGT-M) before transfer. It is recommended for: women above 35, couples with recurrent miscarriages, carriers of genetic disorders, or those with repeated IVF failure. PGT significantly improves success rates and reduces miscarriage risk.

Currently, most standard health insurance plans in India do not cover IVF treatment. However, some employer-provided group insurance schemes and a few private plans are beginning to include limited fertility coverage. We recommend checking your policy directly. Our IVF package typically includes: initial consultation and workup, ovarian stimulation monitoring (ultrasounds + blood tests), egg retrieval procedure, embryology lab fees (fertilization, embryo culture), embryo transfer, and a luteal phase support prescription. 📋 Process & Timeline

A standard IVF cycle takes 4–6 weeks: about 10–12 days of hormone injections, egg retrieval, 3–5 days of embryo culture, embryo transfer, then a 14-day wait before the pregnancy test. With a fresh cycle, you can expect the full process to take 4–5 weeks. A frozen embryo transfer (FET) in a subsequent month takes 3–4 weeks.

Many couples succeed on their first cycle. Statistically, cumulative success rates rise significantly over 2–3 cycles. The number of cycles needed depends on your age, ovarian reserve, sperm quality, and uterine factors. Dr. K.D. Nayar will give you an honest assessment of your likely number of cycles needed based on your workup results.

Before starting IVF, both partners need evaluation: Woman: AMH (ovarian reserve), FSH, LH, Estradiol, TSH, Prolactin, pelvic ultrasound (antral follicle count), HSG (tube check), and general blood tests. Man: Semen analysis, sperm DNA fragmentation test. Some cases may need additional investigations. Dr. Nayar will prescribe only what is clinically necessary.

If you have extra good-quality embryos from your IVF cycle, they are frozen (cryopreserved) using advanced vitrification technology. These can be thawed and transferred in a future cycle. FET cycles are simpler, less expensive, and sometimes have higher success rates than fresh transfers. Our vitrification technology has >95% embryo survival rates.

ERA (Endometrial Receptivity Analysis) is a test that analyses the lining of the uterus to find the exact “window of implantation” — the optimal time for embryo transfer. It is particularly recommended for patients who have had repeated implantation failure despite good embryos. ERA can significantly improve success rates in these cases.

🧬 Male Infertility

Yes. For men with azoospermia (no sperm in ejaculate), sperm can often be surgically retrieved directly from the epididymis (PESA) or testes (TESA/Micro-TESE). The retrieved sperm is then used in ICSI. Dr. K.D. Nayar works with our specialist andrologist to determine the best approach based on the cause of azoospermia.

Sperm DNA fragmentation (SDF) refers to breaks in the DNA strands within sperm. High SDF can lead to fertilization failure, poor embryo quality, recurrent miscarriages, or IVF failure even with normal semen parameters. We offer SDF testing and use Microfluidic Sperm Sorting (MFSS) technology to select sperm with minimal DNA damage, significantly improving outcomes.

PESA (Percutaneous Epididymal Sperm Aspiration) and TESA (Testicular Sperm Aspiration) are minor surgical procedures to retrieve sperm directly from the reproductive tract when no sperm is present in the ejaculate. PESA retrieves sperm from the epididymis; TESA retrieves sperm directly from testicular tissue. Both are done under local anaesthesia and are minimally invasive.

🏆 Success Rates

Our IVF success rate is approximately 65–73% per cycle — among Delhi’s highest. Success rates vary by age: under 35 (70–75%), 35–37 (65–70%), 38–40 (55–65%), above 40 (40–55%). With donor eggs, success rates can be 70–80% regardless of recipient age. Dr. Nayar will give you a personalised estimate during consultation.

Factors that can improve IVF success include: a healthy BMI (18–28), not smoking, limiting alcohol, taking prescribed supplements (folic acid, Vitamin D, CoQ10), stress management through our Emotional Fertility Coaching, and following all medication instructions precisely. Advanced techniques like ERA testing, PGT, and Microfluidic Sperm Sorting also improve outcomes in specific cases.

A failed IVF cycle is emotionally difficult, but it is not the end of the road. Dr. K.D.
